Something may be overriding the V-Sync setting in the game. For example, the AMD drivers have a setting that can disable V-Sync in everything, and there's an "Enhanced Sync" option that can also cause problems in this game. You could have Steam, or something else, display the frame rate while playing, and see if that matches the rate set in the game.
On a related note, I think someone posted that they fixed a problem by limiting the frame rate in the Nvidia(?) drivers. I guess that would have the same effect as enabling V-Sync.
Your video card drivers are probably overriding the game setting. For example, the AMD drivers can be set to disable V-Sync in everything. I'm not familiar with the Nvidia drivers, but I imagine that they have a similar option.
