But you _do_ know that this registration is OPTIONAL, right? It only unlocks internet multiplayer and nothing else. Except that multiplayer registration is pointless since the servers are down.
I never registered my version of the game and it all works fine.
The form you mention was copied from a paper registration card that the boxed version came with back in the day. This is why it has all the funny fields like "address".
This was back in a time, when people went on the internet using 22.8 kbyte Zyxel modems and it would actually be more convenient to drop a printed registration card in a physical mailbox.
I believe when you actually sent the registration info, then Topware awarded you with a free 1024x768 wallpaper for download, if I'm not mistaken. Ah! Those were the days! You still remember those heavy CRTs we had, right? Just think of how proud we were of our clumsy, flickering 17" screen. Oh boy! All those memories... ;)
Seriously: just skip the registration. You will be fine!
