It has the sexual content tag, which I specifically blocked. it even says

"In your discovery because its popular, has tags you filtered sexual content"

Oh that's the problem sorry. Then I'm not sure, maybe because it's not in the top 5 tags on the game or Steam really does not care.

Might want to double check your filters, you may have clicked the filters to look for sexual content games rather than block...or it could be a bug that is doing the opposite of what you want, you may want to let tech support know, they're usually pretty prompt in fixing those from what I've seen.

From what I know of the filtering system on Steam, it works almost perfectly.
There's a few bugs for certain.

I would say that since Sexual Content is normally a user tag, the system is not filtering it properly on your end.
OR you did accidentally have that set to search and not to exclude.

Its definitely set to exclude the tag. but apparently " popularity" just ignores your banned tags.

That's very strange. I've never seen any evidence that popularity actually ignores that.
