That said, you need to have concealment to land a sneak attack.
Undercover doesn't give concealment when you play it (unless you play its discard ability instead). neither does the associate's/crime lord ring.
Is there a minimum concealment requirement for sneak attack to proc, that I have missed? Or some set of conditions in the open world game modes that somehow renders concealment non-functional despite being displayed? It happened in my second run ever in open world game modes, so there's every chance that I overlooked something that would be obvious to an experienced player.
But then concealment will not display under your portrait. If you do have concealment then it's always displayed under your portrait. That's the place to look for it.
The verbose log will be helpful to see if any roll modifiers apply, so it will clear out doubts ie. if the roll is low or if sneak attack doesn't trigger.
In this case damage roll should have been at least 6, so you didn't land Sneak Attack for sure. However I still can't replicate any problem with the perk, so it could be explained logically by not having Concealment at the moment.
Otherwise it might be something more serious. Let me know how it goes, to know if I need to keep looking or not ;)
I might go back and test some more if I think of something I didn't try. I'm kinda miffed about not being able to replicate it. I'll update if I manage it, but if not then I guess I'll have to accept that it might have been user error.
